Breaking Free From the OCD Cycle

Obsessive-Compulsive Disorder (OCD) is more than just a tendency to be neat or organized. It’s a deeply distressing and often exhausting cycle of intrusive thoughts and repetitive behaviors that can interfere with daily life. People with OCD experience unwanted, persistent thoughts, images, or urges (obsessions) that create significant anxiety, along with strong urges to perform certain actions or mental rituals (compulsions) in an attempt to find relief.

OCD takes many forms. You might be plagued by intrusive thoughts about harming someone you love, despite having no desire to do so. You might fear contamination, doubt whether you locked the door, obsess over religious or moral 'wrongness,' or feel compelled to perform mental rituals to neutralize 'bad' thoughts.

If you’re here, you may already have a sense of how overwhelming OCD can feel. You might notice your mind getting “stuck,” your anxiety spiking around uncertainty, or your routines becoming increasingly rigid. It’s important to know that you are not alone, and that effective, evidence-based help is available.

At Equilibrium Psychotherapy, our clinicians specialize in understanding the nuanced ways OCD shows up in people’s lives. We approach treatment with both expertise and compassion, using evidence-based OCD treatment including Exposure and Response Prevention (ERP) to help you break free from the OCD cycle and regain a sense of control, flexibility, and peace. With the right support, meaningful change is not only possible, it’s expected.

We offer in-person and telehealth OCD therapy to clients throughout the greater Madison area, including Middleton, Fitchburg, Sun Prairie, Verona, and Monona, WI—so you can access specialized OCD treatment wherever you are most comfortable.

Signs You May Be Dealing With OCD

You are constantly bothered by unwanted thoughts, images, or urges that pop into your mind and cause you great anxiety.
You have an extreme fear of germs, dirt, or contamination, leading you to wash or clean excessively, often until your skin becomes raw.
You are plagued by disturbing and forbidden thoughts about violence, sex, or religion that you find repulsive or frightening.
You constantly doubt yourself and feel compelled to check things repeatedly, such as ensuring doors are locked, the stove is off, or that you didn't make a mistake.
You feel intense distress if objects around you are not perfectly ordered, balanced, or facing a certain specific way.
You engage in repetitive mental acts like counting, praying, or silently repeating words to try and "neutralize" or cancel out a bad thought.
These obsessions and compulsions take up a significant amount of time (often over an hour a day) and interfere with work, school, or relationships.
You recognize that the thoughts are irrational, but you cannot stop them or the resulting rituals, despite wanting to.

What to Expect in OCD Therapy

Assessment & Psychoeducation

  • We begin by getting to know you as a person and creating a safe, judgment-free space to discuss your experiences.
  • We will map out your specific obsessions, compulsions, avoidance behaviors, and how much time your OCD currently consumes. This will include the completion of the Yale-Brown Obsessive Compulsive Scale.
  • You will receive "psychoeducation" to understand the OCD cycle (thought, anxiety, compulsion, temporary relief) and why compulsions only strengthen the disorder over time.
  • Together, we will set measurable goals and create a collaborative treatment plan tailored to your life and unique symptoms.

ERP Treatment

  • Together we will create a personalized exposure hierarchy (fear ladder). In this activity, you will list and rank your triggers (obsessions) from the least anxiety-provoking to the most challenging, ensuring treatment progresses gradually.
  • Under our clinical guidance, you will slowly and intentionally confront the situations, objects, or thoughts you avoid, both in-session and through homework (exposure).
  • You will learn to resist the urge to perform mental or behavioral compulsions, which helps to break the OCD cycle for long-lasting change (response prevention).
  • We will retrain your brain to tolerate uncertainty. By staying in the anxious situation without performing a compulsion, you learn that anxiety naturally subsides and the feared outcomes are unlikely to happen (habitualization).

ERP can feel challenging, but research shows it produces lasting results. Your therapist will move at a pace that is brave but never overwhelming.

Common Questions About OCD Therapy

What is ERP therapy and why is it the leading treatment for OCD?

Exposure and Response Prevention (ERP) therapy is the gold-standard treatment for OCD. It is a structured therapy where you gradually face your triggers (exposure) while learning to resist the urge to perform mental or behavioral compulsions (response prevention). This process helps you break the obsessive-compulsive cycle, tolerate uncertainty, and retrain your brain for long-lasting change.

I have really disturbing intrusive thoughts. Does that mean I'm a bad person?

Absolutely not. Intrusive thoughts are a defining feature of OCD and are often called "ego-dystonic," meaning they conflict with your true values and identity. Thoughts about harm, sex, or religion that you find repulsive or frightening are unwanted and involuntary. The fact that you are distressed by them suggests you are a good person whose brain is misfiring; these thoughts do not reflect your character.

How is OCD therapy different from regular anxiety therapy?

While OCD is classified as an anxiety disorder, the therapy (ERP) is highly specialized. General anxiety therapy often focuses on reducing anxiety symptoms; however, in OCD, the primary target is the compulsion or ritual. OCD treatment is centered on Exposure Response Prevention, which means deliberately resisting the urge to perform rituals. This stops the cycle of temporary relief and prevents the OCD from growing stronger over time.

Can OCD be cured?

Many experts avoid the word "cured" because OCD symptoms can sometimes flare up under stress. However, OCD is highly treatable. Through ERP, you can achieve long-lasting change, retrain your brain, and stop the compulsive cycle from controlling your life. Our goal is effective management, enabling you to reclaim your life and function without the disorder consuming a significant amount of your time.

Is medication necessary for OCD? Will you support me in communication with my prescriber if I want to explore this option?

Medication is not always necessary, but a combination of ERP and psychiatric medication (like SSRIs) is often the most effective approach for reducing symptoms. If you wish to explore medication, we will absolutely support you by coordinating care and communicating with your prescriber to ensure your treatment is integrated and optimized for your specific needs.

Do you treat OCD in children and teens?

Yes, Equilibrium treats OCD in adolescents and teens, ranging in age from 12-18 years old.

Is OCD therapy covered by insurance?

Yes. OCD therapy is covered under mental health benefits by most major insurance plans. We accept several major insurance carriers. Contact your insurance provider to verify your specific mental health benefits and any co-pay or deductible requirements.
